Attachments - To add attachments to the Form.
Images - To attach images .
Save - To save the form .Initially status of the form will be OPEN .
Click Buttons - To go to the Search window of the corresponding Columns.

Report Code - Generated by System during Save .
Product Group - Lists the Product Groups Available.
Product - List the products based on the selected product Group.
Solution History - Click the right button to view the solution history.It displays the Solution details for the Claim ,updated by LG.

Model - User can select the model from the Model Search window .Click the Button near model to go to search window.The Fig. below shows the Model search window . After entering the search criteria press Search button .Select a model from the list and click Choose button to add the model to the form.

Symptom Codes - Click the Button near Symptom code to go to the Symptom code window. From there user can select the symptom codes and save it.The Fig. below shows the Symptom code list window .

Status Information
After saving the Form , you can view the Status Information at the end of the form.
Status History can be viewed by Clicking on Status History Button.

OPEN          - Initially after save , form will have OPEN status
SUBMIT       - After Submitting the Form to LG by the Agents ,status will be changed to SUBMIT
CANCEL     - When the user cancels the form .
APPROVE   -Claim approved by LG
REJECT        - Claim rejected by LG
CLOSE       - After Approval ,Solution(Parts Supply,S/W Upgrade or Factory Support) will be executed.Solution History will be entered and CLOSE the Form by LG .
ACCEPT    - After CLOSE , the form will be accepted by the Agent .
